• Submit Valid, signed U.S. Passport, must have at least 6 months remaining validity. Passport must contain at least one blank visa page for visa to be issued.
• 1 Fully completed and signed, barcoded Online Visa Application Form.

• 1 Fully completed and signed Authorization Letter

• 2 Passport Photographs
• A business letter of responsibility typewritten on your company letterhead and addressed to: Consulate General of Taiwan, Los Angeles

The letter must explain all of the following:
• Nature of business to be performed
• Name and address of references to be visited,
• Exact duration of stay in Taiwan
• Guarantee of return transportation and sufficient funds
• Letter should be signed by a company officer other than applicant
• Copy of round trip ticket or itinerary
• For Non - US Citizens: A copy of Green Card or Legal Status in United States Should also be Submitted
• Completed Ambassador Passport and Visa Cover Letter
• International Certificate of Vaccination for Yellow Fever is required if Arriving from Infected Area within 5 days
